Create a new MySQL database (or choose an existing one). Create a new user
(or use an
existing user) with access to that database with (at minimum) the following
privileges:
SELECT, INSERT, UPDATE, DELETE, and CREATE.

Lad os sige, at brugeren hedder "mogens", databasen hedder
"tommeflasker", password skal være "hejsahop", og mogens skal have
adgang fra alle *.mydomain.dk hosts:

create database tommeflasker;
grant
   select, insert, update, delete, create
   on tommeflasker.*
   to mogens@"%.mydomain.dk"
   identified by "hejsahop"
